Messages are placed in the queue by qmail-queue. Once a message has been dropped off, it's considered "queued". Next, qmail-send "preprocesses" the messages. Preprocessing consists of deciding which recipients are local and which are remote, and sometimes rewriting recipient addresses. See the INTERNALS file for more details.
