Can I scan SMTP Email within Qmail?

Sure can! Try qmail-scanner (http://qmail-scanner.sourceforge.net/).
This is an anti-virus "harness" specifically written for Qmail. It replaces
qmail-queue (performance!) and allows you to use almost any Unix-based
commercial virus scanner (inbuilt, ClamAV, Trend, MacAfee, Sophos,
H+BEDV, and more) to scan all your Qmail deals with (both incoming and
outgoing). Typically it would be used on an Internet SMTP gateway to
protect the Internet from your users (oh yeah - and the reverse! ;-)
It also supports blocking by attachment type - useful for those sorts of
sites who want to stop their lusers swallowing their Internet link with
MP3 files/etc!
