To add a guestbook to your site it really depends on what your server 
supports.  There are many free guest-book applications out there as 
well as a few dedicated servers just for guestbooks and hosted 
applications.
Many older guestbooks used CGI scripts (Perl) to gather the data for 
the guestbook and parse it into and HTML page that the user could see 
and you could administer.  Many new ones are written in PHP, ASP, JSP, 
or ColdFusion which gives greater flexibility to the scripting and what 
you can do with it.  
One of the most common ways is to set up a form and have that form 
insert into a table in your database.  This requires a little scripting 
knowledge and database knowledge.  See the faqts link above for general 
reference information on building a DB driven application.
I would suggest that you find out what your host supports, see if they 
have a guestbook application available or can recommend one, if not 
check out some hosted applications (guestbooks not on your server).
