You appear not to have the premissions on your MySQL server set up to 
let you access a database.
A mysql command like:
   GRANT ALL ON *.* TO 'apache'@'localhost' identified by ''
will probably fix your problem. However you may want to set up a 
password for the apache user (or use a different user to access the 
database). The documentation of mysql_connect 
(http://www.php.net/mysql_connect) should help you with this.
