I get 'permission denied' on trying to create or append to a file under NT, why?

It seems that the directory or the file itself is not defined as
shareable. Also, the directory may be protected and permissions might
have been granted to just a few users.
To find this, Open Explorer, go to the directory under question, and
right click on it. Click on 'properties'. Under Security tab click on
permissions and confirm that you are mentioned there as a user allowed
to access.
Follow the same for the file which you need to modify / create.
