Application Form for new passport duly filled in. Current passport and
additional booklets, if any, issued subsequently. Four latest passport
size photographs, one photograph to be pasted on the application form
and signed across. Duly notarized affidavit from the applicant giving
reasons for change of appearance to be signed in the front of Justice of
Peace. Affidavit must also bear old and new photographs. A fee of
A$78.00. Minimum processing time for issuing new passport due to change
of appearance is 35 days.
