I have access to Linux, FreeBSD & all Windows versions. Which OS should I use as my MySQL/PHP host?

I would advise you to use a unix based operating system for hosting 
purposes, like what you have access to (freebsd and linux) are very 
much camable of hosting large websites with php. Mysql also runs 
perfectly on unix based systems. Therefor use unix based OS's for you 
production machines because in performance they are better.
Weather or not you should use linux or freebsd is 'a matter of taste', 
as far as I know there are not many performance difference although 
many think that freebsd is more stable (I have no  numbers), so choose 
to use the one you are most familiar with / the one your friends / 
gurus know.
